Supporting Connections - SAVVI

The SAVVI program is offered to pension-level residents of  Supported Residential Services

Supporting Connections Workers  operate in a manner that is:

• Client centred
• Proactive
• Responsive
• Targeted
• Collaborative
• Capacity building
• Consolidate service structures developed by the SAVVI Facility Cost Relief Project

Aims of the program

• Identify unmet resident health and social needs and assist them to access appropriate services.
• Build skills/capacity of proprietors and staff to identify resident needs, improve care planning and better manage challenging behaviours.
• Build skills/confidence of residents to access services and participate in activities as independently as possible.
• Strengthen links between pension-level Supported Residential Services and local services including developing new service approaches where appropriate.
